2021 USOPHE Conference Sponsorship and Exhibitor Registration

USOPHE is seeking sponsorship of the 2021 conference to be held virtually on November 4, 2021. This year's conference theme is:

Growing Together Toward Tomorrow

Thank you for your interest in contributing as a USOPHE Conference Sponsor. We appreciate you for helping provide professional development and networking opportunities for Utah health education professionals.

Donations are due by August 30, 2021. Any donations made prior to June 30, 2021 will automatically be part of our early bird special. In addition to the incentives listed below, early birds will receive a verbal recognition at our summer webinar event, recognition on our social media, and your logo will also be added to the summer webinar agenda. (Payments not received by these dates will not be eligible for the donation incentives.)

Contribution Levels and Benefits:

$2,000 - Platinum Level

• Verbal recognition for being a sponsor before the opening and closing keynote speakers day of the conference
• Full page ad in our digital "Sponsor Booklet" sent via email to all the attendees with the conference agenda
• 5 complimentary conference registrations
• Special shout out on our social media

$1,500 - Gold Level

• Verbal recognition for being a sponsor before the opening and closing keynote speakers day of the conference
• Half page ad in our digital "Sponsor Booklet" sent via email to all the attendees with the conference agenda
• 4 complimentary conference registrations

$1,000 - Silver Level

• Verbal recognition for being a sponsor before the opening and closing keynote speakers day of the conference
• Quarter page ad in our digital "Sponsor Booklet" sent via email to all the attendees with the conference agenda
• 3 complimentary conference registrations

$500 - Bronze Level

• Verbal recognition for being a sponsor before the opening and closing keynote speakers day of the conference
• Logo will be placed in our digital "Sponsor Booklet" sent via email to all the attendees with the conference agenda
• 2 complimentary conference registrations

$100 - Friends of USOPHE

• Verbal recognition for being a sponsor before the opening and closing keynote speakers day of the conference
• Logo will be placed in our digital "Sponsor Booklet" sent via email to all the attendees with the conference agenda
• 1 complimentary conference registration

USOPHE Wildcard (miscellaneous donations, i.e. another monetary amount, gift cards, swag, products, etc) Please contact USOPHE Treasurer (usophetreasurer@gmail.com) to donate the materials or amount.

In the undesirable event that the 2021 USOPHE Conference is cancelled, any sponsorship donations will be used to support other public health projects or an alternative event.

Please note any participant in the sponsorship agrees to indemnify, hold harmless, and defend (and pay any and all other expenses and attorney’s fees, in connection therewith) USOPHE or its agent from and against any and all liability, loss, claims, and/or actions arising out of or relating to any alleged unauthorized use of any patent, trademark, design, or copyright provided to USOPHE to use in connection with the sponsorship activity.
